The Health & Wellness Center at Moravian Hall Square is a licensed nursing home for long term skilled nursing care and short term rehabilitation.

The Centers for Medicare and Medicaid (CMS) rate the quality of every licensed nursing home in the country and have given The Health & Wellness Center at Moravian Hall Square the highest possible rating: 5 stars.

Moravian Hall Square is also the only continuing care retirement community in the Lehigh Valley region that has been accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF). Accreditation from CARF is the golden seal signifying the highest standards for quality care and service have been achieved. There are thousands of continuing care retirement communities in the United States…but only one in Lehigh Valley, PA has been accredited by CARF.

One of the many ways the Health & Wellness Center at Moravian Hall Square excels is the fact it has its own Medical Team onsite daily and on-call 24/7. Through a unique partnership with the Senior Health Center for Positive Aging at St. Luke’s University Health Network, the medical team provides services for the physical, emotional, social and functional needs of patients by coordinating care with the patient’s own primary care physician, onsite medical specialists and MHS’ nursing team. Members of our Medical Team include: 

Dr. Eileen Simak, DO – Dr. Simak specializes in the health care needs of older adults. She is a Board Certified Physician in Internal Medicine and Geriatric Medicine, and earned her degree from the Philadelphia College of Osteopathic Medicine and a Master’s of Business Administration from St. Joseph’s University. Working in collaboration with other medical specialties, such as orthopedics, cardiology and behavioral health, Dr. Simak provides a comprehensive approach to managing the health care needs of residents at Moravian Hall Square. 

Annette Frederick, CRNP – Annette is a Certified Registered Nurse Practitioner. Previously she was a trauma, orthopedic nurse at St. Luke’s University Hospital in Bethlehem, PA. She has a Master’s Degree in Adult Gerontology Primary Care from Moravian College in Bethlehem, PA. 

Desheil Pearsall, MA – Desheil, Medical Assistant, attends the University of Phoenix and will graduate with a Bachelor’s Degree in Criminal Justice and Human Services. Prior to joining the Senior Health Center for Positive Aging Desheil worked in the New York City Police Department.
